We own and operate seven skilled nursing facilities statewide, as well as four independent living facilities and one assisted living facility, providing a wide range of services within warm and supportive atmospheres where residents are cared for according to their individual needs, preferences and lifestyles.

Skilled Nursing
Our seven skilled nursing facilities strive to meet you where you are, with compassionate, person-centered support designed to help you retain your independence as much as possible, and provide the comfort, connection and familiarity that makes you feel right at home. Programs include:
Short-Term Rehabilitation Care: Comprehensive and rehabilitative therapy and skilled nursing care for individuals following hospitalization, helping them get back on their feet and back home as soon as possible, with full functional ability and reduced risk for setbacks.
Long-Term Care: 24/7 nursing and customized supportive services for individuals with chronic illness, with personalized care plans helping them achieve their highest level of independence and live life to the fullest every day in new and exciting ways.
Memory Care: A safe, supportive and highly structured environment for individuals living with Alzheimer’s disease and other forms of dementia, enabling them to live their best life while honoring their daily routines, needs and preferences to the fullest extent possible.
Palliative Care: Specialized care for individuals with serious long-term illnesses, helping to promote the dignity of life and reduce pain, stress and discomfort.
Respite Care: Short-term care and support within an engaging and secure living environment for individuals whose primary caretakers require temporary relief.
Independent Living
Service-enhanced, independent living for individuals seeking to maintain their independence and enjoy life on their own terms. Our communities offer spacious living, modern amenities and many social opportunities, within secure and comfortable apartment settings. Services include access to on-site meals and dining, light housekeeping, transportation and recreational activities.
Assisted Living
A combination of spacious housing, around-the-clock support services and personal care for individuals who require limited assistance within a close-knit community. We offer the same level of support and features you’ll find at more expensive assisted living facilities, including homecooked meals, housekeeping, laundry, transportation, exercise and wellness programs, as well as various social and recreational services.
